The Urine Pregnancy Test (UPT) detects human chorionic gonadotropin (hCG) — the 'pregnancy hormone' produced by the developing placenta shortly after a fertilised egg implants in the uterine wall — in urine. hCG appears in urine as early as 10–12 days after conception, making the UPT detectable from the first day of a missed period in most cases. Important Note: A positive UPT indicates hCG in urine — most commonly from pregnancy, but rarely from hCG-producing tumours (choriocarcinoma, gestational trophoblastic disease, or certain germ cell tumours) or exogenous hCG administration. A negative UPT does not exclude very early pregnancy if tested before hCG reaches detectable levels — repeat testing after 48–72 hours or use serum beta-hCG (more sensitive) if clinical suspicion is high. All positive results should be followed up with clinical assessment, serum beta-hCG, and ultrasound.
UPT vs. Serum Beta-hCG — When to Upgrade: The UPT is a qualitative test — it answers 'pregnant or not pregnant' but gives no information about how much hCG is present. Serum beta-hCG (Test 79) is a quantitative test — it measures the exact hCG level in blood. Serum beta-hCG is preferred when: (1) very early pregnancy is suspected but UPT is negative; (2) ectopic pregnancy is suspected (hCG level + ultrasound findings guide management); (3) monitoring pregnancy viability — doubling of beta-hCG every 48–72 hours confirms a healthy intrauterine pregnancy; (4) post-abortion/miscarriage monitoring of hCG clearance.
