The Urine Protein/Creatinine Ratio (UPCR) is a highly accurate, non-invasive test that quantifies how much protein is being lost through the urine by comparing protein concentration to creatinine concentration in a spot urine sample. This ratio eliminates the effect of urine dilution and provides a reliable estimate of 24-hour proteinuria — the clinically accepted alternative to the cumbersome 24-hour urine collection. It is a critical marker for detecting and staging kidney damage.
Key benefits of this package include:
What Does the Protein / Creatinine Ratio — Urine Measure?
| Level / Result | Possible Interpretation |
|---|---|
| < 0.2 mg/mg (Normal) | Normal kidney filtration — no significant protein in urine |
| 0.2–0.5 mg/mg (Mildly elevated) | Borderline proteinuria — may indicate early kidney disease or benign causes; repeat and monitor |
| 0.5–1.0 mg/mg (Moderate proteinuria) | Significant kidney damage — likely CKD, diabetic nephropathy, or hypertensive nephropathy |
| 1.0–3.5 mg/mg (Heavy proteinuria) | Substantial kidney damage — nephrology referral and further workup recommended |
| > 3.5 mg/mg (Nephrotic-range) | Nephrotic syndrome — massive protein loss; urgent specialist evaluation required |
Important Note: UPCR should be interpreted alongside kidney function tests (serum creatinine, eGFR), blood pressure, HbA1c, and clinical history. A single elevated result should be confirmed with a repeat test, as transient proteinuria can occur with fever, exercise, or urinary tract infection.
UPCR vs. Urine Microalbumin: UPCR measures total urine protein — including albumin and other proteins. Urine Microalbumin specifically measures albumin in the urine at low concentrations and is the preferred test for early diabetic nephropathy screening, where albumin leakage precedes overt proteinuria. For established proteinuria quantification and CKD staging, UPCR is the standard. For early-stage screening in diabetics, Microalbumin (or Albumin/Creatinine Ratio — ACR) is preferred.
