The Coagulation Package is a comprehensive panel of tests that together evaluate all major aspects of the blood clotting system — primary haemostasis (platelet plug formation), secondary haemostasis (coagulation cascade), and the time taken for wounds to stop bleeding and blood to form a stable clot. This panel is the standard pre-operative coagulation screen and the first-line investigation for any patient with unexplained bleeding, bruising, or a family history of bleeding disorders. No fasting required.
Key benefits of this package include:
What Does the Coagulation Package Measure?
| Level / Result | Possible Interpretation |
|---|---|
| All parameters normal | Normal haemostatic function — no evidence of bleeding disorder in coagulation screen |
| Low platelets + Normal PT + Normal APTT | Isolated thrombocytopenia — dengue, ITP, hypersplenism; platelet plug formation impaired |
| Prolonged BT + Normal PT + Normal APTT + Normal platelets | Platelet function disorder (thrombocytopathy) or von Willebrand disease — further specialised testing needed |
| Prolonged APTT + Normal PT + Normal platelets | Intrinsic pathway defect — haemophilia A or B, Factor XI deficiency, heparin therapy, lupus anticoagulant |
| Prolonged PT/INR + Normal APTT + Normal platelets | Extrinsic pathway defect — Factor VII deficiency, early warfarin effect, early Vitamin K deficiency |
| Prolonged PT + Prolonged APTT + Low platelets | DIC pattern, severe liver disease, or combined coagulopathy — urgent evaluation required |
| All parameters prolonged + Very low platelets | Severe DIC or bone marrow failure — medical emergency |
Important Note: The Coagulation Package is a screening panel — abnormal results direct further specific investigation (factor assays, von Willebrand studies, D-dimer, fibrinogen). Normal results do not guarantee no bleeding risk for all procedures. The BT component requires an in-centre visit. All results must be interpreted by a physician or haematologist in the full clinical context.
DIC — When the Coagulation System Turns Against Itself: Disseminated Intravascular Coagulation (DIC) is a life-threatening condition where uncontrolled activation of the coagulation system causes simultaneous widespread clotting (consuming clotting factors and platelets) and paradoxical bleeding (from factor/platelet depletion). It complicates sepsis, severe trauma, obstetric emergencies, malignancy, and extensive surgery. The Coagulation Package signature of DIC is: prolonged PT + prolonged APTT + low platelets + low fibrinogen + elevated D-dimer. Early recognition through coagulation testing is critical for life-saving treatment.
